"Please click again to confirm" issue

when I try to register , login, submit question I got this error , nothing is working on my q2a page

What do you mean by "submit media"? The clearer the question, the better the answers. Also, can you reproduce it in the demo site? http://demo.question2answer.org

i mean question, i think it's because of the hosting provider , but they don't help to me, if I know the problem I will tell them

1. What do server logs say?
2. Does it happen to all questions? Do any of them contain a <form> tag inside them?
3. Do you face this issue without having any plugin in your qa-plugin folder?
4. Do you face this issue while using the Classic theme?

1- server logs is in answer
2- it's happen all the time, i can not submit any question , i can not login, can not register,
3- I tried
4- I didn't  change anything , using default files from q2a
