I am having trouble with my new install of q2a 1.7.4

For logged in users everything works well. But since I have set voting to logged i users only and answering to anyone,

when a visitor tries to answer then the message please click again to confrim shows repeatedly.

when a visitor tries to vote, instead of showing 'please login to vote' it shows 'please reload the page and try again'

i have disabled all plugins and using default theme and the only error i found in logs was

[Fri Mar 25 12:07:31.137634 2016] [:error] [pid 1659] [client 127.0.0.1:34148] PHP Fatal error:  Call to a member function read_post() on a non-object in /var/www/ask/qa-include/app/format.php on line 1935, referer: /ask/1775/when-will-the-black-panther-rise

Which editor are you using? Seems like it's missing a function.

Also can you look in the Developer Tools in your browser, and see if there are any errors in the Console?

I do not see any console errors when clicking the vote button as anonymous. Ideally it should show please login or register to vote. But it shows "Please reload the page".

Same case with submitting an answer as a visitor. No console errors before or after submitting. After submitting, the page is reloaded and the "please click again to confirm" message is shown below the answer text area with the previously submitted answer showing as an editable field.
