How to use "#" in Questions and Answers before the word so it will become a tag automatically?

Just like Twitter and Instagram, you put # before the words and they will become tags automatically. Users can click on #whatever and see what's under that tag. Is there any plugins or features you already have or can anybody give me some hint about this?

I also needed a similar functionality, so I created the Hashtagger. The only difference, that hashtags will work not only for questions and answers, but also for comments. If you want to disable this, in file qa-hashtagger.php find and remove the filter_comment() function.
