How to create pages with custom question and custom title/url with pagination?

where i can enter question ids 1,2,3..... and a title for page.

in admin/pages -> add page
we have fields for title, position, heading and Content to display in page.
I'm also want one more field to enter the question ids if we click on check box -> add custom questions.
