Need Guidance to create Custom Page with a Form to accept data

Hello,

  I am a newbie, trying to create a custom page. Any guidance on the below will be helpful.

• I want to create a custom page with a form ( it has few fields).  Registered users should be able to submit the form and the results should be stored in database. 
• I want to query the database and display all the stored records in the form of a table. Everyone should be able to see the table. 

I am new and trying to customize my Q2A to have this feature. If there are any existing plugins, direct me to them. If not, suggest me where to start...
