Just to note down one error that I got (and similar too):

PHP Question2Answer form security violation for ask by anonymous (code mismatch) on /ask via domain.com&gws_rd=cr&ei=PrjAVvvCDYKfyAOvia7YBw

Just wondering if the "&gws_rd=cr&ei=PrjAVvvCDYKfyAOvia7YBw" is triggering the security form?

I guess google scanned the page, got an "old" security code and somehow it is pushed to the server?

This must be a cached page and which have a old security code on the form and q2a treats as a expired security code .
