Avatar on login bar not visible on some admin pages

Question

I just realized that avatar on login bar not visible on some admin pages. For instance Genral, users etc. but visible on plugin, moderate, stats, flagged and hidden.

I checked URL for image (blob id) that is always the same on every page. Just wonder what's wrong?
