PHP Warning: session_start() - The session id is too long or contains illegal characters

Question

From my current error logs:

PHP Warning:  session_start() [<a href='function.session-start'>function.session-start</a>]: The session id is too long or contains illegal characters, valid characters are a-z, A-Z, 0-9 and '-,' in /qa-include/qa-app-users.php on line 143

How can that happen?

Here is a php snippet to prevent those errors, do we need to include this in q2a?
