How do I fix these W3.org errors?

I checked my pages with the Html and Css validator at W3.org and found a few errors, but repeated many many times:

How do I fix them?

Some hints:

• You have two <title> tags specified, you should only have one
• Your <meta> tag uses curly quotes, should be straight quotes like ". Also missing an = sign. Should be <meta name="description" instead of <meta name”description”
• Don't end meta tags with /> as you are using HTML not XHTML.

The above is not Q2A code, it's custom code so it's either something you added or is coming from a plugin.
