What does it mean and how to fix it?

This error is being caused in one of your plugin layers, quite possibly by the Mouseover Layer plugin that comes with Q2A. What page is being requested when this error is shown?

when old pages requested this error occured

OK, try adding an @ sign before $question['title'] in qa-mouseover-layer.php
