Approving users is a manual action by the admin/moderator. Whereas email confirmation is an action by the user themselves to prove their email address is valid.
It's up to you what you do, but I think most sites would not want to use manual approval since its time consuming. It's useful if you want to restrict who can register, or as a temporary measure of you get hit by spam sign ups.
