Is there a custom page CONTACT FORM plugin??

Question

Is there any contact page plugin which can add a contact from on a custom HTML page. This feature is normally used in wordpress on contact us page, so I think if anyone would have made anything like this.

I know that feedback page is there but having a custom page contact plugin would be a great addon to q2a.

Actually I want some more pages just like feedback page which could have contact form. Those pages can be like Support page, contact us page.

amiya I asked @sama55 about it, but he said that the plugin is for feedback page, not for custom pages. After that only I raised a question here.
