required field and checkbox on register page

I need a checkbox while register user and this check box must be validate.

My hack (Extra userfield) may serve as reference, too. I revised code for 1.6.2 the other day.

Hey sama! Good job  for your hack but even if I'm able to customize my (test) site, i'm not able to set an extra field as mandatory... any suggestion?
