On set-up I'm unable to receive "OK" on Adminastration URL Structures

Question

After I've installed Q2A, the URL structure "OK" doesn't load. I'm unable to change the URL structure. The permissions look right (755), I believe my server version and software are okay. What am I doing wrong? see attachment.

 

URL structure: 

• /123/why-do-birds-sing (requires htaccess file)
•  /index.php/123/why-do-birds-sing
•  /?qa=123/why-do-birds-sing
•  /?qa=123&qa_1=why-do-birds-sing
•  /index.php?qa=123&qa_1=why-do-birds-sing

Server versions and software:

• PHP : version 5.4.43
• Apache: version 2.2.29
• MySQL : 5.1.65
• MySQLi : 5.1.65

This is a very strange problem.  Have you solved it yet, have you tried to reinstall?
